Free printables
Free logs and checklists you can print today
No sign-up and no email. Print a sheet, or save the image to your phone. They’re blank forms: you write the numbers, and your doctor says what they mean.
The sheets
Pick one, print it, use it this week.

Blood pressure log sheet
Date, time, upper and lower numbers, pulse and notes. Room for 22 readings.

Blood sugar log sheet
Date, time, before or after a meal, the reading in mg/dL and notes. Room for 22 readings.

Medicine list
Each medicine, how much, when (empty stomach, before or after food, bedtime), what for and which doctor. For a parent or for you.

Doctor visit checklist
What to take, questions to ask, and what to check before you leave the clinic.

Emergency Medical ID card
A fold-up wallet card: blood group, allergies, long-term conditions, doctor and two people to call. One for each person.

Symptom diary: one week
A few words a day on what you felt, when, and how strong, to describe the week to your doctor.
Using them well
A few honest notes.
The sheets only record what you write. They don’t say what a number means or what to do about it. Ask your doctor how often to measure and which numbers suit you.
You’re welcome to copy and share them, for free, as they are. If you keep readings in the Kinsu app instead, you can export them as a spreadsheet file to show your doctor.
